Jacob is committed to serving the wholistic health needs of the adult and geriatric LGBT populations. He began with CrescentCare in 2007 as a Community Specialist, became a case manager in 2009, and then a primary care nurse in 2016.

His goal is to become a primary care provider for our aging LGBT adults, to raise awareness for their unique needs, and to facilitate community programs to improve overall health outcomes.

• Currently pursuing DNP of AGPCNP with specialization in Oncology, expected graduation May 2020.
